Physicians CareConnection has a medical clinic for the uninsured and underinsured that is free or low cost if they need to be referred out for further services. We provide pregnancy support by connecting women to their first prenatal appointment and connecting them to resources families may need in preparation for their babies. Our Housing Stabilization program helps to connect people to resources if they get behind in their rent and utilities or help them think through their options for stable housing. Our Engagement Specialist provides addiction support for people to discuss options for recovery and get connected to services, including pregnant women. Care Coordinators provide family coaching and support to those who need peer support, linkage to food services, social service referrals, employment support, education support, and self-care support. Translation services are available.
